Scheduling an appointment
Clients are seen by appointment only. Appointments are scheduled Monday through Saturday. Evening and Saturday hours vary by counselor.
Completing the paperwork
After scheduling your first appointment, you will need to complete the New Client paperwork. We provide several options to make this easy for you.
- You may choose to come in 15 minutes before your appointment to fill out a copy in our office.
- You may request that we send you a secure, encrypted email with a copy that you can fill out and return using the same secure email.
- You may request that we send you a secure, encrypted email with a copy that you can fill out and bring with you the day of your appointment.
Arriving for your appointment
Please be sure to arrive 15-20 minutes before your appointment to sign the Consent to Treatment and Health Insurance Portability and Accountability Act (HIPAA) forms.
If you are filing insurance, be sure to bring your card with you. If you are under 18 years of age, a parent or guardian MUST attend the first session with you in order to sign paperwork.
Insurance
We accept most major insurance providers. Please talk with our office staff prior to your appointment to verify whether your insurance will be accepted.
We do not accept Medicare or Medicaid.
Payment
Payment is expected in full at the time of service. This includes any co-pays, co-insurance, or deductible amounts.
Accepted Methods: Cash, Check, Visa, Mastercard, Discover, AMEX (including payments made from your HSA, HRS, or FSA).
